Students will be screened for the following:

Vision

• Grade: K, 1, 2, 3, 4, 5, 7, 10
• Grade 1: Color Deficiency

Hearing

• Grade: K, 1, 2, 3, 4, 5, 7, 10

Scoliosis

• Grade: 5 girls
• Grade: 7 boys & girls
• High school follow-up on referrals
• Information Sheet

Any additional screen will be done by request. Parents will be notified of students who do not pass screening.
